Jacob was trained at the Rijks Universiteit Groningen as Doctoral Business Economics with a main focus on Systems Approach and informatics.
He started his career in Health Care at the University Hospital Leiden with the development of the BAZIS Integrated Hospital Information System.
In 1979 he became project manager of the first DRG project in the Netherlands. Together with Prof JM. Rodrigues he started the International Network for Patient Classification Systems (international network for health care management strategy). Since 1990 he is a member of EFMI and serves as president of this federation since 2008.

In 2005 he left HISCOM to be more active in the world of Casemix in the Netherlands, as consultant at the ministry of health and special adviser of the Dutch casemix office. Since 2006 he became involved in the application of casemix tools on chronic disease management for Diabetes care to support the ZonMW experiment. At the Dutch casemix organization he played an important role in the design of the second version of the DBC system, the so-called DOT. In 2009, he joined the project team of the introduction of the Integrated Funding system at the ministry of Health.
